Patient Zero is a post-Moon Lord boss that is fought in the final section of the Abandoned Lab.
While Patient Zero is alive, the music Xenostate will play.
Spawn
Patient Zero is found dormant in the final room of the Abandoned Lab named "Sector Zero". Damaging its eye will intiate the fight after a short build up.
To resummon Patient Zero after defeating it, the player needs to use the Hologram Remote in Patient Zero's arena.
Behavior
When the fight starts, Patient Zero will remain dormant for a bit, waiting for the music to build up. Setting the music volume to 0 will begin the fight immediately.
The fight with Patient Zero is divided into multiple phases. Every second phase Patient Zero becomes invulnerable and opens its lower part, revealing Kari Johansson. After Kari's phase is over, he becomes invincible instead, switching to Patient Zero. Patient Zero retains its attacks from previous phases, while Kari's attacks always change depending on the phase.
If the player defeats Ancient Deity Duo, Patient Zero will gain 25% damage reduction. Whenever Patient Zero performs an attack that includes Xenium Lasers, its defense will increase to 80.
Patient Zero will close its eye and become immune to damage if there are tiles blocking its line of sight to the player.
Phase 1 (Patient Zero 100%-75% health)
- Fires 4 Xenium Lasers in a plus shape, as well as 12 Caustic Tears in a circle.
- Launches 3 Xenium Blasts directly at the player. The Blasts split into 4 Xenium Miniblasts upon impact.
- Fires 4 Xenium Lasers in an X shape, as well as 12 Caustic Tears in a circle.
- Launches 4 Xenium Blasts in random directions.
- Fires 2 pairs of horizontal Xenium Lasers that rotate in a way that forces the player to stay above or below Patient Zero. During this attack, Patient Zero also shoots Caustic Tears directly at the player.
- Launches 2 Xenium Blasts to the left and right of the player, followed by a Xenium Blast directly at them.
Phase 2 (Kari 100%-75% health)
- Launches 4 Tear of Infection Balls that split into 2 Tears of Infection on impact.
- Fires 6 arcing Poison Beats in random directions.
Phase 3 (Patient Zero 75%-50% health)
- Fires 4 rotating Xenium Lasers while shooting Caustic Tears in 4 rotating directions.
- Launches 5 Xenium Blasts directly at the player.
- Fires 2 horizontal Xenium Beams that slowly rotate before switching their direction. During this attack, Patient Zero fires Tears of Pain directly at the player, as well as in the opposite direction.
- Launches 3 Xenium Blasts at the player, then a singular Blast, then 3 Blasts again.
- Fires 4 pairs of Xenium Lasers in an X shape that slightly converge on each cardinal direction. Patient Zero also launches 4 Tear of Pain Balls that split into 2 Tears of Pain upon hitting a tile after bouncing once.
- Launches 5 Xenium Blasts in random directions.
Phase 4 (Kari 75%-50% health)
- Launches 4 Caustic Tear Balls that split into 2 Caustic Tears on impact.
- Fires 6 arcing Poison Beats and 4 arcing Infectious Beats in random directions.
- Fires 4 Xenium Lasers aimed directly at the player.
Phase 5 (Patient Zero 50%-35% health)
- Shoots 8 lasers in all cardinal and ordinal directions, with 5 out of the 8 sections created by lasers being barraged by Xenium Miniblasts. The affected areas will be highlighted before the lasers are fired.
- On the For the worthy seed the telegraphs last half as long.
- Launches 2 bursts of 4 Xenium Blasts.
Phase 6 (Kari 50%-25% health)
- Launches 4 Tear of Pain Balls that split into 2 Tears of Pain on impact after bouncing once.
- Fires 8 arcing Poison Beats and 4 arcing Infectious Beats in random directions.
- Fires 7 Xenium Lasers aimed directly at the player.
Phase 7 (Patient Zero 35%-10% health)
Uses all previous attacks at random.
Phase 8 (Kari 25%-0% health)
Uses the attacks from the previous phase.
Aftermath
- Star Serpents start spawning in the Space layer.

- Patient Zero takes 33.3% damage from Last Prism and 40% damage from Empyrean Blind's orbs.
Trivia
- The Bestiary entry for Patient Zero: "An unfortunate scientist, mutilated and disfigured by the Xenomite infection. This specimen was Kari Johannson, the father of all T-Bots and patient zero of the xenomite infection. He's been stuck for 50 years, conscious and aware of the situation around him... God, that must be tormentous."
- The term "patient zero" refers to the Index case or the first patient infected in an epidemic.
